GLP-1RAs, such as semaglutide (Ozempic®) and tirzepatide (Mounjaro®), mimic the hormone GLP-1, enhancing insulin secretion and regulating appetite. SGLT2 Inhibitors, like Farxiga® and Zynquista®, prevent kidney reabsorption of glucose, promoting its excretion in urine. These agents have demonstrated cardiovascular and renal protective effects in people with T2D and show promise for those without diabetes.

Traditional oral agents, such as metformin, remain essential, while lifestyle modifications, including diet and exercise, are critical components of managing T2D alongside pharmacotherapy.

The artificial pancreas technology is rapidly advancing toward a fully automated system, which would function without any input from the user—a major advancement beyond current "hybrid" systems. Current commercial hybrid closed-loop insulin delivery systems, such as Medtronic’s MiniMed 670G, 770G, and the more advanced 780G, adjust insulin automatically but still require user input for meal announcements and exercise adjustments.

The field is focused on creating a fully automated closed-loop artificial pancreas (AP) system, which would manage blood sugar levels without any user intervention. Early trials with fully closed-loop systems show promise in managing unannounced small meals and may eventually eliminate the need for meal announcements.

Future innovations such as once-weekly insulin and regenerative therapies like beta cell replacement via stem-cell derived cells are promising but still emerging, primarily in the context of type 1 diabetes. These advancements, however, offer insight into strategies that might ultimately influence treatments across diabetes types.

This evolving landscape aims to enhance effectiveness, convenience, and safety for people living with T2D, with technological and pharmacological innovations working hand in hand. The future of diabetes treatment is undoubtedly exciting, with promising developments on the horizon.
